CAMP & HIGH ADVENTURE PHYSICALS

Dr. Parker will attend the April 22 troop meeting to provide FREE physicals to Scouts.

There are two mandatory requirements for scouts to receive a physical on April 22nd.

1) A parent or guardian must be present with the scout during the physical.

2) Part A General Information of the new Health and Medical Record form must be completed prior to the start of the physical. Dr. Parker will complete Part B during the physical. The forms are available online at: http://www.stcbsa.org/campinfo/NewHealthForm2009.pdf

IMPORTANT: Scouts attending Sea Base, Northern Tier, Philmont, or NOAC must bring the correct Health and Medical Record form for these events and must have the General Information portion completed.
